Linking primary care to population health: Behind the Primary Care Home model

At this year's UK e-Health Week, Nav Chana, Chair of the National Association of Primary Care, spoke to HIMSS International Chief Clinical Officer (CCO) Dr Charles Alessi about improving population health outcomes through digital.

“There needs to be a big cultural change to enable our colleagues across the system to be very comfortable with digital technology, understand its power and its benefits, and to embrace it and not challenge it, because, actually, without that culture of learning and support, it’s not going to work,” Mr Chana told the new HIMSS CCO.
